Some Effikal brand vent dampers (involving all sizes) that were manufactured between July 1, 2007 and March 1, 2008 have developed cracks on the plastic ears connecting the motor assembly to the mounting bracket. If both ears break, the motor assembly could disconnect from the bracket. If this occurs, the boiler may light with the vent damper in the closed or partially closed position.

Vent Damper Replacement Procedure:

1. Purchase replacement motor assemblies, Effikal Part #70009000, for $25 from the wholesaler.
2. Replace vent damper motor assemblies of all vent dampers in the
   affected serial number range OR date code.
3. The replaced part must be returned in the box provided to
   2630 Airport Road, Kinston, NC 28504, using the prepaid UPS
   label provided in the box. 
4. Include the completed Replacement Payment Form and a copy
   of your invoice. 
5. Effikal will reimburse you $125 per job ($100 for labor + $25 for part).

This replacement program involves Effikal brand PC vent dampers only. 
The Effikal motor assemblies affected have black plastic bases and black plastic caps.
The vent damper must be within the serial number range OR date code as listed in the chart below.

Use of this Vent Damper Replacement Payment Form is a requirement for reimbursement. By signing this form, the servicing contractor acknowledges that the Vent Damper Replacement Instructions have been read and followed. Contractors will be paid $100 for labor for each replacement within the appropriate date code range and/or serial #, along with $25 reimbursement for the purchase of the replacement motor assembly, for a total of $125.

Payment will only be made after receipt of the following in the box provided:

1. The replaced vent damper motor assembly with cap.
2. This completed and signed Replacement Payment Form.
3. A copy of your invoice detailing the work performed.

And it is verified that the date code and/or serial # of the vent damper is
part of the program.

vent damper in the box provided. Please allow 4-6 weeks to process payment.
